The rape and murder of Jill Meagher in September last year by a convicted sex offender who was out on parole has been described as the most catastrophic failure of the Victorian justice system. The Parole Board failed to act when notified by police that Adrian Bayley had been arrested over a vicious assault on a man in Geelong in August 2011, creating a series of events which ended in murder.
